Medical Social Work

The Medical Social Workers at Neo Home Health apply their advanced expertise to conscientiously counsel patients and their families on all relevant health issues. Our professionals will also thoroughly explain all in-home health care policies and resources, assist patients and their loved ones in locating and utilizing additional treatment services when advisable, and help facilitate an open dialogue between our patients and their medical team. Specific services that may be provided by our employees include:

• Observation, documentation and reporting of clinical and treatment information, including patients’ behavioral changes

• Comprehensive coordination of prescribed services with other available resources

• Referral of patient or family members to community support resources

• Arrangement of meals and transportation after hospital stay or discharge from care facility

• Assistance with adapting course of treatment in accordance with changes in a patient’s condition and recommendations from the medical team
